Mary Carson Breckinridge was an American nurse midwife who dedicated her life to improving healthcare in rural and impoverished regions of the United States. She is best known as the founder of the Frontier Nursing Service (FNS), an organization established to provide comprehensive family medical care to the mountain communities of rural Kentucky. Operating in remote areas that lacked road or rail access, Breckinridge and her staff famously traveled by horseback to reach patients in need. Breckinridge modeled her healthcare delivery system on European midwifery practices. She advocated for the professionalization of American nurse-midwives, promoting a system where they could practice autonomously within decentralized clinics and directly in patients' homes. Her efforts yielded significant results, dramatically lowering infant and maternal mortality rates in Appalachia at a relatively low cost. Despite the documented success of her model, her vision of autonomous nurse-midwifery did not become widely adopted across the United States.
